Camper Health History form is required for all campers...
A completed IE Camper Health Form must be on file with the gym for campers to participate. Please bring your completed form with you on the first day of camp. Thank you!!

Medical, Medication, and Asthma Forms are only required for those that require those items.
Unfortunately, we can't reuse school and daycare forms as they not permitted for summer camp use per state policy.

International Elite requests all routine medications be provided before or after camp and only emergency medications (i.e... asthma inhalers or epipens) are able to be provided during camp.
Please bring your completed forms with you on the first day of camp. Thank you!!
